About The Position

Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The provider will conduct examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. The position requires managing acute and chronic illnesses across all ages within a family unit in a community setting, encompassing health promotion, illness prevention, and a holistic healthcare model. Responsibilities include managing chronic diseases, acute illnesses, coordinating care with specialists, and utilizing community resources. The role advocates for access to healthcare with a focus on structured evaluation, early intervention, and health promotion. Consultation with supervising physicians or team members is expected for complex cases. The provider will work within and promote the Patient-Centered Health Home model of care, ensuring excellent customer service and patient education. Participation in daily huddles, performance improvement efforts, chart and peer reviews, and potentially supervising students/residents are also key aspects of this position. Adherence to attendance policies and flexibility to work at any FHCN location are required.

Requirements

  • Completion of an accredited Nurse Practitioner or Physician Assistant training program, with a minimum cumulative GPA of 2.5
  • Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
  • Minimum credit score of 650.
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.
  • Licensed in the state of California as a Nurse Practitioner and/or licensed in the state of California as a Physician Assistant.
  • Effectively communicate professional opinions and extrapolations of information collected, synthesized/analyzed.
  • Determine appropriate methods of communicating information through the use of tables, graphs, charts, and other visual forms.
  • Preparation and execution of presentations to large groups.
  • Regularly required to sit and use repetitive hand movements to type and grasp.
  • Frequently required to stand or walk.

  • Must be able to assist the patient care team with performance improvement efforts, staff education, and FHCN promotion.
  • Must be able to assist the patient care team with performance evaluations and quality improvement.
  • Must meet Network and credentialing expectations for licensure and employment.
  • Must adhere to the Attendance and Absenteeism Policy.
  • Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.

  • Offer comprehensive health care including health promotion and maintenance, prevention of illness and disability, and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
  • Manage chronic diseases and acute illness with a broad range of knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
  • Advocate access to health care with a focus on structured evaluation, early intervention, and health promotion.
  • Consult with supervising physician or other team members on cases that need further input.
  • Work within and help to promote the Patient-Centered Health Home model of care.
  • Provide excellent customer service and care, behaving courteously when interacting with patients/family members, support staff, and Network staff.
  • Remain aware of wait time during the daily workflow.
  • Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
  • Ensure patient status as measured by relevant clinical quality measures.
  • Promote and participate in daily huddles.
