Advanced Practice Provider Outpatient Palliative Care
About The Position
This role focuses on providing outpatient palliative care. The Advanced Practice Provider (APP) will develop and implement individualized care plans in collaboration with physicians, nurses, and other healthcare providers. Responsibilities include managing medications and treatments to alleviate pain and distressing symptoms, providing emotional and psychological support to patients and their families, and educating them about illness progression, treatment options, and advance care planning. The APP will also coordinate care with primary care providers, specialists, social workers, and therapists, advocating for patients' needs to ensure holistic, patient-centered care. The position offers an excellent work/life balance, with workdays Monday to Friday, including call responsibilities, but no weekend work.
Requirements
- Master’s degree or higher in nursing or physician assistant studies from an accredited program
- Current Illinois license as a nurse practitioner or physician assistant
- Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
- Minimum of two years of experience as an APP in primary care or a related field required
- Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
- Proficiency in electronic medical record (EMR) systems
