About The Position

The ICAR unit at Inova Schar Heart and Vascular is looking for an Nurse Practitioner/Physician Assistant (APP) to join their team! This role involves providing and managing pre and post-procedural patient care for various cardiac and vascular procedures. The APP will assist in ensuring timely patient discharge and unit flow, collaborate with anesthesia and proceduralists, and manage post-procedure orders and care. They will serve as the primary support for RNs regarding patient needs and complications, allowing physicians to maintain their schedules. Responsibilities include managing low-dose chronotropic and vasopressor initiation, arterial and venous hemostasis, hemodynamic management, arrhythmia management, pain management, electrolyte management, airway management in the PACU, and reviewing CXR for pneumothorax. The role also involves coordinating emergent care and transfers, ordering routine follow-up procedures for IR, ordering follow-up CT scans, and assisting with patients requiring admission from ICAR until handed off to floor staff. This includes managing patients who become septic after a tube change, have volume overload requiring diuresis, or experience complications requiring overnight stays. The APP will coordinate care with other teams to ensure smooth and safe transitions of care and utilize POCUS for bedside echocardiograms to evaluate procedural complications. Requirements

  • Graduation for an accredited NP or PA program.
  • Must be approved to practice as a Nurse Practitioner or Physician Assistant in the state of Virginia.
  • Current prescriptive privileges and DEA license required.
  • BLS and ACLS for Healthcare Provider from AHA required with expiration date greater than 90 days from date of beginning practice.
  • Nurse Practitioner: Master’s Degree or Doctorate of Nursing Practice required.
  • Nurse Practitioner: License to practice as a Registered Nurse required.
  • Nurse Practitioner: Certification in practice from a nationally recognized certifying body required.
  • Physician Assistant: Graduate from an accredited Physician Assistant program required
  • Demonstrate appropriate critical thinking and interpersonal skills when working in a fasted paced, high acuity work environment.
  • Able to perform a thorough history and physical assessment.
  • VA License
  • Certification - Basic Life Support prior to start; Advanced Cardiovascular Life Support (for Critical Care) upon start; Pediatric Advanced Life Support (for Pediatrics) upon start
  • Licensure - Licensed Physician Assistant or Nurse Practitioner
  • If Nurse Practitioner: RN Compact License(depending on State of primary residence)
  • If Nurse Practitioner: AND a license (or eligibility for) as a nurse practitioner or physician assistant in the Commonwealth of Virginia
  • If Nurse Practitioner: AND a "Practice Agreement" signed by sponsoring/ supervising physician(s)
  • If Nurse Practitioner: AND a written "Scope of Practice/Protocol" signed by the supervising physician
  • If Nurse Practitioner: AND "Drug Enforcement Administration License" (if prescribing controlled substances).
  • For Nurse Practitioners: Bachelor's Degree in Nursing and Master's Degree in Advanced Clinical Practice or graduate of a post-masters certificate program for nurse practitioners, OR certificate from an education program that is accredited by the Council of Nurse Anesthesia Educational Programs/Schools, American College of Nurse Midwives, American Association of Colleges of Nursing of the National League for Nursing
  • For Physician Assistants: Graduate of an accredited Physician Assistant program

Nice To Haves

  • ACNP, AGACNP certification preferred based on population served.
  • Physician Assistant: Master’s Degree preferred.
  • Certification from the National Commission on Certification of Physician Assistants preferred.
  • EPIC experience preferred
  • No prior experience required; APP experience highly preferred.

Responsibilities

  • Provide/manage pre and post procedural patient care for EP, Interventional cardiology, structural heart, vascular and IR procedures
  • Assist with ensuring timely discharge/transfer from ICAR, unit flow and utilization
  • Work with anesthesia and proceduralists in pre procedure, PACU and recovery as appropriate to manage patient care
  • Placing post procedure orders and care management orders for inpatients and outpatients
  • Primary support/contact for RNs for patient needs/complications/care pre and post procedures
  • Allows physicians to stay on schedule and perform planned procedures – communicates with physicians as needed
  • Including/not limited to: Chronotropic and vasopressor low dose initiation and management
  • Arterial and venous hemostasis
  • Hemodynamic management
  • Management of arrythmias
  • Pain management
  • Electrolyte management
  • Air way management in PACU
  • Review of CXR/rule out pneumothorax
  • Assist in coordination of emergent care and transfer to appropriate level of care/team
  • Order routine follow-up procedures for IR (nephrotic, biliary or abscess tube exchanges)
  • Order follow-up CT scans if necessary
  • Help with patients requiring admission from ICAR until handed off to the floor staff
  • Coordination of care with other teams, orders, level of care, etc. providing smooth and safe transition of care for patients
  • Working on POCUS for bedside echocardiogram, rule out procedural complications as part of evaluation of events, symptoms
