About The Position

Puzzle Healthcare is a mission-driven, national post-acute care organization focused on protecting patients during their most vulnerable moments—right after a hospital stay. As an Nurse Practitioner/Physician Assistant in Sapulpa, OK, you’ll provide focused care in nursing facilities helping reduce readmissions and improve recovery while enjoying autonomy, a predictable schedule, and strong team support. This is an onsite position serving one or more skilled nursing facilities (SNFs) in the Sapulpa, OK area.

Requirements

Nurse Practitioner or Physician Assistant
Ability to provide focused care in nursing facilities
Ability to reduce readmissions and improve recovery
Ability to work autonomously
Ability to work a predictable schedule
Ability to work with strong team support
Ability to serve one or more skilled nursing facilities (SNFs)
Ability to work weekday daytime hours
Ability to work without call or weekends
Ability to complete documentation using Puzzle's EHR
Ability to monitor readmission risk
Ability to maintain compliance with post-acute and PM&R-related regulations

Responsibilities

Round in assigned facilities on a consistent weekday daytime schedule
Oversee daily patient interactions through timely micro visits that support focused assessments to assess recovery, manage pain, wounds, and functional decline
Collaborate with physicians, therapists, and facility staff on individualized recovery plans
Use Puzzle’s EHR to complete documentation (often remotely from home)
Monitor readmission risk and help close gaps in post-acute care
Maintain compliance with post-acute and PM&R-related regulations
