Requirements
- Advanced Practice Provider: Nurse Practitioner (FNP) or Physician Assistant (PA)
- Current South Carolina licensure or eligibility for licensure
- Current Basic Life Support (BLS) certification
- DEA and applicable state prescribing credentials prior to start date
- Commitment to evidence-based medicine, patient-centered care, and continuous learning
Nice To Haves
- Primary Care, Family Medicine, Internal Medicine, or Urgent Care experience preferred
Responsibilities
- Provide comprehensive primary care services for patients across the lifespan
- Diagnose and manage acute and chronic medical conditions
- Conduct annual wellness visits, preventive screenings, and routine health assessments
- Develop individualized treatment plans focused on improving long-term health outcomes
- Provide patient education and counseling on disease prevention and healthy lifestyle practices
- Coordinate care with specialists and multidisciplinary healthcare teams
- Utilize evidence-based clinical guidelines and best practices
- Participate in quality improvement and patient safety initiatives
- Collaborate within a supportive physician and APP team environment
