About The Position

We are seeking a dedicated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing primary care team in Houston, TX. This opportunity is ideal for an APP who is passionate about delivering high-quality outpatient care in a collaborative, patient-centered environment. We are especially seeking providers who can connect with our diverse patient population, with Mandarin-speaking or Spanish-speaking providers. You’ll work alongside physicians, care managers, behavioral health specialists, and support staff to provide comprehensive care focused on prevention, chronic disease management, and long-term wellness.

Requirements

  • Master’s or Doctorate degree from an accredited Nurse Practitioner or Physician Assistant program.
  • Active Texas license as one of the following: Nurse Practitioner (FNP) Physician Assistant (PA-C)
  • Active DEA license (or ability to obtain prior to start).
  • National certification applicable to role/specialty.
  • Strong commitment to patient-centered and culturally competent care.

Nice To Haves

  • Fluent in Mandarin or Spanish
  • Experience in Family Medicine, Primary Care, Internal Medicine, or Pediatrics.
  • Experience working in diverse or underserved communities.
  • Value-based care or population health experience a plus.

Responsibilities

  • Evaluate, diagnose, and treat patients in an outpatient primary care setting.
  • Conduct wellness visits, preventative screenings, chronic disease management, and acute care visits.
  • Develop and implement individualized treatment plans.
  • Order and interpret diagnostic tests, labs, and imaging.
  • Educate patients on medications, treatment plans, nutrition, and preventative health.
  • Support patients in managing chronic conditions such as diabetes, hypertension, and asthma.
  • Promote continuity of care through proactive follow-up and patient engagement.
  • Collaborate closely with physicians and interdisciplinary care teams.
  • Coordinate referrals, diagnostics, and specialty follow-up care.
  • Participate in a team-based, integrated care model focused on quality outcomes.
  • Maintain accurate and timely documentation in the EHR system.
  • Ensure compliance with clinical guidelines, regulatory standards, and quality initiatives.
