About The Position

COPC is the largest independent physician-owned primary care group in the nation. We are seeking an experienced full-time licensed Advanced Practice Provider to join our team at Knightsbridge Internal Medicine, 4830 Knightsbridge Blvd., Suite A, Columbus, Ohio 43214. This practice has 4 Physicians, 3 Medical Assistants, 1 Clinical Lead, 3 Patient Service Reps, and an Office Manager to support the physicians and patients. This COPC practice has earned a reputation for being patient-centered. The culture of the practice supports one another. They provide the best primary care for all ages with a focus on health education, disease prevention, and management to keep their patients well. COPC uses state-of-the-art EHR - EPIC. Training is available during onboarding. Office Hours: Open Monday-Friday, 7:30 am - 4:00 pm. Central Ohio Primary Care was founded in 1996 when a group of 33 physicians joined together to give greater focus to patient care than to the administrative duties that were weighing them down. From the onset, COPC physicians have always been dedicated to finding new ways to offer the best patient care. Today, we have over seventy-seven practices, a team of hospitalists, Same Day centers, and ancillary services. Our goal is to provide the highest quality preventative and chronic care and education to keep patients out of the hospital whenever possible. Requirements

  • Master’s degree in nursing or physician studies from an accredited program.
  • Current licensure as a Nurse Practitioner or Physician Assistant in the state of Ohio.
  • National certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA).
  • Strong clinical assessment, diagnostic, and treatment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.

  • EPIC experience is helpful.

Responsibilities

  • Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages.
  • Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries.
  • Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, and educate patients on self-management strategies.
  • Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.
  • Collaborate with physicians, advanced practice providers, and nurses to coordinate patient care across multiple disciplines and healthcare settings. Facilitate referrals to specialists and other healthcare providers as needed.
  • Maintain accurate and timely documentation of patient encounters, treatment plans, and follow-up care using electronic health record systems in compliance with regulatory standards and organizational policies. COPC uses EPIC.
