About The Position

Cleveland Clinic's Taussig Cancer Institute is seeking a qualified Physician Assistant or Nurse Practitioner for the Palliative Medicine Program at Cleveland Clinic Mercy Hospital. The role involves conducting new consults and follow-up visits, rounding with attending physicians and other team members, and managing adult patients on the division. Care includes symptom assessment and management, serious illness conversations, and goals-of-care discussions. The position is mostly based in the inpatient clinic, with occasional weekend and holiday coverage. The Advanced Practice Provider delivers primary and/or specialty care in collaboration with the healthcare team and in accordance with certification and licensure. The work emphasizes health promotion, disease prevention, and diagnosis and treatment of acute and chronic health problems. The provider also contributes to education and professional development across the care team. APP experience is preferred; new graduates with extensive oncology, ICU, or hospice experience may be considered. APRNs must obtain ACHPN certification in Hospice and Palliative Care within the first two years of hire.

Requirements

For APRN roles: graduate of an accredited APRN program; an MSN is required upon hire or within three years of hire for applicable external hires and internal hires moving from a non-APRN licensed role to an APRN role.
For PA roles: graduate of an A.R.C.-approved Physician Assistant program and hold a master's degree in Physician Assistant Studies or a higher degree from a program recognized by the applicable state licensing board.
For NP roles: current Nurse Practitioner and RN licenses in applicable state(s), and national Nurse Practitioner certification from an organization recognized by the state Board of Nursing.
For CNS roles: current Clinical Nurse Specialist and RN licenses in applicable state(s), and national Clinical Nurse Specialist certification from an organization recognized by the state Board of Nursing.
For PA roles: certification through the NCCPA or successor organization recognized by the applicable state licensing board; a license to practice and prescribe; and a current DEA license or DEA designee as required by NCQA.
Basic Life Support (BLS) through the American Heart Association is required; additional life-support certification may be required depending on the work setting.
Nurse Practitioners and Clinical Nurse Specialists must have arrangements with at least one physician in accordance with applicable State Board of Nursing requirements.
Physician Assistants must have a Physician Supervision Agreement and signed addendum.
Must be credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.

Responsibilities

Conduct new consults and follow-up visits and round with attending physicians and other team members.
Manage adult patients, including symptom assessment and management, serious illness conversations, and goals-of-care discussions.
Conduct medical histories and physical examinations where indicated; initiate appropriate tests, review results, and develop working diagnoses and treatment plans.
Perform and interpret diagnostic procedures and perform routine or specialized procedures as credentialed and privileged.
Prescribe and monitor medications as permitted by licensure and applicable policies.
Educate patients about their conditions, treatments, and related medication or treatment side effects and hazards.
Collaborate with nursing, medical, and other healthcare team members, including participating in medical team rounds.
Refer patients to specialists and connect them with appropriate healthcare resources and treatment.
Support continuity of care by coordinating with patients and multidisciplinary care teams or specialty areas.
Review records and treatment plans to monitor outcomes and help assure quality care.
Support appropriate length of stay, discharge planning, and compliance with inpatient regulatory standards.
Support professional development of the healthcare team and promote evidence-based practices, guidelines, and research.
May conduct research studies, assist with data collection and analysis, precept students, and onboard new caregivers.
Perform other duties as assigned.
