About The Position

We’re hiring an Advanced Practice Provider (NP or PA) - Float who wants a predictable weekday schedule, variety in their workday, and the stability of an established physician-led primary care organization. You’ll step into an outpatient role focused exclusively on adult primary care, providing coverage across established Jacksonville-area clinics. You’ll manage acute and chronic conditions, preventive care, wellness visits, medications, diagnostics, and referrals with support from a multidisciplinary care team.

Requirements

• Board Certified Nurse Practitioner or licensed Physician Assistant
• At least 2 years of adult primary care experience
• Comfortable independently managing acute and chronic conditions
• Value collaborative, team-based outpatient care

Nice To Haves

• Medicare Advantage experience
• Spanish proficiency
