The Lead OB Advanced Practice Provider for Digital Care (OnCall Connect OB) is a member of a collaborative team responsible for delivering high-quality virtual, digital, and community-based maternity care to a diverse population of patients across the Ministry using technology in the patient’s home environment and, as needed, in-person care. Within scope of practice, the Lead APP performs comprehensive assessment, diagnosis, and intervention, utilizing evidence-based care and clinical decision support to guide practice and promote safe, consistent, patient-centered outcomes. In addition to direct patient care, the Lead APP provides clinical leadership for a group of advanced practice nurses by reviewing and monitoring clinical quality in coordination with the Medical Director and Clinical Directors; supporting the design, implementation, and tracking of practice quality/outcome indicators; and ensuring consistent orientation and clinical practices across digital health programs. The Lead APP partners with the Director, Manager, and Clinical Director for OSF OnCall Digital Health, and collaborates with physicians, advanced practice providers, and other care team members to drive service excellence, engagement, access, and cost-effective, efficient care delivery, including conducting regularly occurring meetings to facilitate information dissemination and team support. We are looking for a candidate with OB experience that is passionate about community healthcare and the OB population. The OnCall Connect OB APN is responsible for providing virtual prenatal and postpartum care. This is a hybrid position with one to two days per month of care on our Mobile OB Unit. OnCall currently supports OB care within OSF by offering virtual visits for the missed menses, first prenatal and two-week postpartum visits for patients who see most OSF OB practices. We will also serve as the primary OB provider for Medicaid patients on a mobile OB unit. OnCall Connect OB also offers virtual support to the Medicaid population through a collaboration with the state. These patients are provided an extra layer of support through an application that is monitored by the RN team. Occasionally these patients require escalation visits after triage. The APN will be responsible for these same day virtual visits. The role of this position will continue to evolve, and change based upon the needs of the community.
