Advanced Practice Provider (PA/APRN) serves as both a provider and chronic disease educator, supporting patients across multiple outpatient clinics with a focus on diabetes, metabolic disorders, and primary care integration. Will collaborate with family medicine providers, clinical dietitians, and case managers to care for patients, provide diabetes-specific interventions, and improve care outcomes. Responsibilities include advanced assessment, medication management, patient education, nutritional and lifestyle counseling, and care coordination. This role enhances the depth and scope of care available to high-risk populations by bridging medical treatment with education and preventative strategies. Will also lead initiatives to improve population health metrics, provide staff education, and participate in outreach to underserved populations. Is expected to understand documentation and billing workflows that support diabetes care and chronic care management services.
