Advanced Practice Provider - Diabetes and School Health (DASH)
About The Position
Diabetes and School Health (DASH) is an enhanced care-coordination program that works to provide the best diabetes care through direct oversight, education and support for patients living with Type I and Type II Diabetes. Services are provided in schools. This position will travel to schools in Columbus and the surrounding area to provide services to students enrolled in the program. Schedule is M-F Day shift. Some options available for a modified summer schedule based on clinical need. Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines.
Requirements
- Master of Science degree or equivalent provision.
- Licensed as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in the state of Ohio.
- Maintains national certification as APRN in the appropriate population foci for the position, or PA.
- Valid Ohio driver’s license and proof of auto insurance is required by hospital policy.
- Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines.
Responsibilities
- Provides healthcare to individuals or groups that requires knowledge and skill obtained from advanced formal education, certification and clinical experience.
- Practices within professional scope and promotes patient advocacy in accordance with the profession’s Code of Ethics and maintains a current Ohio Advanced License.
- Assesses patients and/or populations; develops, implements and evaluates plans of care; clearly documents and communicates information both in written and verbal forms.
- Performs diagnostic and therapeutic procedures according to established guidelines and standards as necessary for the care of the patient.
- Utilizing evidence based practice, serves as a clinical expert for policies, procedures, documentation, and skills.
- Assists in the education, training and development of new and existing staff members.
- Engages in the “community of the work unit” by staying aware of changes, participating in data collection or auditing, engages in Quality Improvement.
- Leverages technology and informatics to optimize healthcare outcomes.
- Commits to continued professional development of self and others.
- Contributes to the division and/or specialty goals and the Hospital Strategic Plan.
