About The Position

The Cardiology Advanced Practice Provider (APP) is responsible for providing both inpatient and outpatient services. In the inpatient setting, the APP works as a team with the attending cardiologist to perform and staff initial consults, subsequent follow up visits and to coordinate patient care. In the outpatient setting, the APP will see patients independently in the cardiology clinic. Outreach clinic may also be expected. Performs other duties as assigned.

Requirements

  • Completion of an accredited Physician Assistant or Nurse Practitioner program
  • Current registration to practice as a Physician Assistant in Minnesota
  • Current national certification by the National Commission on Certification of Physician Assistant
  • Approved by Minnesota Board of Medical Practice for prescription writing privileges
  • Currently licensed as a APRN by the Minnesota Board of Nursing
  • Currently licensed as a RN by the Minnesota Board of Nursing
  • AANP certification
  • Current BLS, ACLS, and DEA certification
  • Licensed as a PA with the Minnesota Board of Medical Practice
  • NCCPA certification
  • Ability to perform as part of a team
  • Ability to communicate effectively both orally and in writing, with peers, students and other health care personnel
  • Ability to work dependently and independently
  • Ability to manage multiple simultaneous tasks and prioritizes appropriately
  • Ability to navigate an electronic medical record to obtain information and document care

  • Experience as an independently practicing APP
  • 1-2 years previous Cardiology experience

Responsibilities

  • Coordinates flow of Inpatient Service
  • Reviews daily patient lists.
  • Prioritizes consultations and rounding schedule according to established parameters.
  • Develops and reviews work plan for the day with rounding physician partner.
  • Communicates regularly with partner Inpatient APP to coordinate consult services and to balance workloads.
  • Assists in the assignment of residents and students as necessary.
  • Discusses and develops diagnosis and treatment plan with staff cardiologists.
  • Assesses and completes H&P on new consults as selected/assigned.
  • Orders diagnostic testing as indicated.
  • Evaluates treatment for assigned patients and communicates information to physician partner.
  • Designs follow-up plan for patients to be seen in the clinic when discharged from the Hospital.
  • Documents assessment, treatment, evaluation and follow-up plan in the electronic medical record.
  • Discusses with and instructs patient/family members regarding treatment and follow-up
  • Interacts with patients and family members to involve them in the plan of care.
  • Educates patient/family members regarding diagnosis and treatment and documents this in the electronic medical record.
  • Provides written instructions when indicated.
  • Prescribes medications as indicated.
  • Evaluates interventions and plan of care regularly to ensure efficacy.
  • Regularly reviews results of ordered diagnostics and communicates these to the partner cardiologist.
  • Reviews patients on the service at the end of the day with physician partner.
  • Modifies plan as indicated.
  • Signs outpatients on service to APP partner when cross-covering.
  • Interacts with other healthcare professionals.
  • Communicates hospital course and treatment plan to referring primary care physicians upon discharge.
  • Cardiology Clinic
  • Evaluate and treat patient in the cardiology clinic on a regular basis. Sees revisits independently and staffs new patients with the NIVL cardiologist.
  • Prescribes medications, orders tests and procedures as needed.
  • Documents clinic visits in the electronic medical record.
  • Regularly manages in-basket, following up on the results of testing and procedures ordered for clinic patients.
  • Communicates with other healthcare professionals regarding mutual patients.
  • Collaborates and coordinates with other health professionals
  • Communicates hospital course and treatment plan to referring primary care physicians upon discharge.
  • Makes and accepts referrals to/from, consults with, cooperates with and refers to all other types of healthcare providers, including primary and specialty care physicians, nurse practitioners and other allied personnel.
  • Discriminates between situation requiring independent management, collaborative management with physician or further referral
  • Collaborates with physician when unusual incidents or changes in patient condition occur.
  • Coordinates care involving other healthcare providers.
  • Functions as mentor/resource and educator for other healthcare team and staff.
