About The Position
Exciting opportunity to Join Endeavor Health’s Center for Cardiac Health–Heart Failure team in an autonomous role delivering patient-centered care to high-acuity patients under the direction of a collaborating physician! Key responsibilities include: Obtaining comprehensive patient histories and performing physical examinations Ordering and interpreting diagnostic tests Developing differential diagnoses and prescribing evidence-based pharmacological and nonpharmacological treatments Managing acute and chronic conditions Providing ongoing education to patients and their families Coordinating care with physicians and interdisciplinary teams to optimize outcomes and support post-discharge follow-up and transitional care Supporting quality-improvement and patient-safety initiatives Contributing to the development and implementation of organizational and service-line goals Adhering to all Endeavor Health policies, procedures, and standards.
Requirements
- Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required
- Federal DEA and Illinois Controlled Substance License required
- Completion of master’s level program in specialty area
- BLS/CPR certification required
- Prior APN/PA Cardiology experience required
- APP experience in cardiology required
Nice To Haves
- Experience working with Heart Failure patients preferred
