About The Position

Cahaba Medical Care is seeking a mission-driven Advanced Practice Provider to provide high-quality, patient-centered care to individuals and families in the communities it serves. The provider assesses, diagnoses, and manages patients within their scope of practice; collaborates with physicians and the multidisciplinary care team; and supports accessible, coordinated, and culturally responsive healthcare. The position may include hosting or supporting learners as appropriate to the provider’s experience and clinic needs. Responsibilities may vary by assignment and may be adjusted based on organizational needs, clinical setting, and applicable laws and regulations.

Requirements

• Certified Nurse Practitioner or Physician Assistant with an active Alabama license and credentials required for the position.
• Ability to provide care within applicable licensure, certification, and scope-of-practice requirements.
• Ability to meet applicable credentialing, privileging, and organizational requirements.
• Excellent communication and interpersonal skills.
• Strong clinical judgment, assessment, and problem-solving abilities.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Commitment to patient-centered care, confidentiality, and professional ethics.
• Strong attention to detail, organization, and follow-through.

Nice To Haves

• Relevant clinical experience is preferred.
• Experience in outpatient care or primary care is a plus.

Responsibilities

• Evaluate patients, obtain medical histories, perform physical examinations, and assess health needs.
• Develop and implement appropriate treatment plans within the provider’s training, licensure, and scope of practice.
• Order, review, and interpret diagnostic tests and coordinate follow-up care as appropriate.
• Prescribe medications and provide treatment in accordance with applicable laws, regulations, and organizational policies.
• Provide preventive care, health education, counseling, and chronic disease management as appropriate to the clinical setting.
• Maintain accurate, timely, and complete patient records and clinical documentation.
• Coordinate care with physicians, nursing staff, behavioral health, and other members of the multidisciplinary team.
• Support quality improvement, patient safety, and organizational clinical initiatives.
• Help identify barriers to care and contribute to efficient clinical workflows.
• Provide compassionate, culturally responsive care to diverse patient populations, including underserved communities.
• Participate in required meetings, training, and other duties related to the position.
• Host or support learners from time to time, as appropriate to the provider’s experience and clinic needs.
