About The Position

Sante Health System is seeking a dedicated Advanced Practice Provider (APP) to join one of our busy medical offices in Fresno, California. We welcome applications from experienced Nurse Practitioners and Physician Assistants who are committed to providing exceptional patient care in a collaborative, physician-led environment. The APP is an essential member of our healthcare team specializing in Gastroenterology. They provide patient-focused care, actively listen to patient concerns, obtain complete medical histories, and perform complete physical exams. The APP collaborates with physicians to manage acute and long-term medical needs, rounds on patients at the hospital, and assists physicians in complex surgical cases. They interpret and integrate data, order diagnostic and therapeutic procedures, and review lab and radiology results in consultation with the supervising physician. The APP also submits patient health care plans and goals for review and provides prescriptions for medications. Additionally, they counsel patients and families on health promotion, diagnosis, prognosis, and management options, recommend community resources, and educate patients and families on medications and treatment instructions.

Requirements

  • Graduate of an accredited Nurse Practitioner or Physician Assistant/Associate program.
  • Current California licensure as a Nurse Practitioner or Physician Assistant in good standing.
  • Minimum of 1 year of experience.
  • Professional clinical experience in a clinic setting is preferred.
  • Exceptional people and communication skills.
  • Highly skilled in multi-tasking in a fast-paced environment.
  • Commitment to achieving a high level of accuracy and attention to detail.
  • A no-task-too-small approach to the work.

Nice To Haves

  • GI experience preferred.
  • Bilingual in Spanish.
  • Previous Gastroenterology (GI) experience is preferred, including experience caring for patients with digestive health conditions in an outpatient clinic, endoscopy center, or hospital setting.

Responsibilities

  • Provide patient-focused care, actively listen to concerns, obtain complete medical history, and perform complete physical exams.
  • Accurately record findings and identify patient care issues that must be addressed.
  • Collaborate with physicians to manage acute and long-term medical needs of patients.
  • Round on patients at the hospital and assist physicians in complex surgical cases.
  • Interpret and integrate data and order laboratory studies, x-rays, and other diagnostic and therapeutic procedures as indicated.
  • Review lab and radiology results in consultation with the supervising physician.
  • Submit patient health care plans and goals for periodic review and evaluation by the physician.
  • Provide prescriptions for medications to accomplish each patient's treatment plan.
  • Take calls for pharmacies and assist the MD when on-call.
  • Counsel patients and their families on health promotion, diagnosis, prognosis, and management options.
  • Recommend community resources to meet patient and family needs.
  • Instruct patients and authorized family members on medications and treatment instructions in an easy-to-understand manner.
  • Educate patients on healing and enjoying a better quality of life.
