About The Position

The Division of Pulmonary, Allergy, and Critical Care Medicine is seeking a Nurse Practitioner for the Pulmonary Vascular Disease Clinic. The NP provides specialized, high-quality care to patients with complex disease, focusing specially on care of pulmonary hypertension, pulmonary embolism and hereditary hemorrhagic telangiectasias. This role functions as a key clinical provider and care coordinator, working collaboratively and closely with physicians, nurses, and multidisciplinary teams. NP plays a critical role in improving access to specialty care, enhancing care coordination, and supporting the growth and clinical excellence of the Pulmonary Hypertension program. You will conduct comprehensive evaluations of patients, titrate and manage advanced PH therapies, interpret diagnostic studies and provide longitudinal follow-up care for patients co-managed with Attending physicians. Additional roles include serving as a point of contact for patients navigating complex treatment regimens, educating patients and families on disease process, treatment plans, and medication management, and providing support and counseling overall. This role directly impacts patient outcomes, strengthens collaboration between clinical team members and contributes to the growth of the Pulmonary Hypertension Accredited Center program.

Requirements

  • Nurse Practitioners should be board certified or board eligible for certification in acute care
  • Eligible or licensed in Colorado (would need to obtain CO license before working)
  • Certified to see adolescents and adults

Nice To Haves

  • General Internal Medicine knowledge base (advanced lung disease and pulmonary vascular disease knowledge and skills are desirable but not required as they will be obtained as on-the-job training).
  • Pulmonary Medicine and Subspecialty background preferred, but not required.

Responsibilities

  • Serve as sub-investigator and work closely with the physicians within Pulmonary Hypertension Clinic to care for patients enrolled in clinical trials
  • Enrollment and follow-up of patients in clinical trials
  • Participation in multi-disciplinary conferences and presentation of patients within these same conferences
  • Outpatient assessment and care of patients with pulmonary vascular disease; assisting physicians in the care of this same population
  • Engaging and communicating with consulting services and with primary care providers
