The APN conducts trauma informed integrated health outreach, screenings, assessment/evaluations, orders, care management, linkages and medication evaluations for individuals eligible for rural preventive health initiatives, including mobile clinic services. Works with multidisciplinary team, community pharmacy and partners to outreach and engage individuals into care and treatment. Provide preventive chronic disease screenings, brief intervention/health education services and referrals/linkages to primary care, behavioral health including substance use disorder treatment. Aids in formulating diagnostic impressions based on DSM-V criteria in collaboration with the psychiatrist, therapist and other treatment members. Develops care plans for individuals requiring ongoing care management including health coaching, specialty care, medication screening and adherence support. May practice in the role of therapist, providing individual and/or group psychotherapy, as needed. Can write prescriptions for non-controlled substances. Will be in verbal and/or electronic contact with the supervisory psychiatrist and clinical management team. Attends meetings as required. Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related needs of the consumers served. The individual demonstrates knowledge of the principles of growth and development appropriate to the consumer population served. He or she must be able to assess and interpret data about the consumer’s status in order to identify each consumer’s needs and provide the appropriate care, including age-related care, needed by the consumer group. Legacy Treatment Services’ Rural Health Transformation Program is a mobile outreach initiative designed to improve access to behavioral health, substance use disorder, telehealth, and coordinated primary care services for underserved rural communities in New Jersey. This program operates in Atlantic and Cape May Counties. The program deploys interdisciplinary mobile outreach vans into rural and rural coastal areas to provide screening, intake initiation, social determinants of health assessment, medication support, prevention services, and rapid linkage to ongoing outpatient and primary care treatment. The initiative is anchored within Legacy’s licensed outpatient infrastructure, allowing mobile services to function as an extension of established clinical programs rather than as standalone care. The project aims to reduce wait times, improve continuity of care, address barriers such as transportation and poverty, and prevent escalation to crisis or emergency department utilization through early intervention and coordinated community-based care.
