About The Position

Great new Career Opportunity Now Hiring - Advanced Practice Nurse - Teaneck NJ, Oradell NJ. The Advanced Practice Nurse will be responsible for conducting visits to patients in accordance with local, state and federal regulations, communicating visit findings to designated members of the interdisciplinary team, facilitating access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment), consulting with other members of the interdisciplinary team to contribute to the patient's plan of care, preparing and maintaining accurate patient records, charts and documents, and assisting in the development of clinical practice guidelines/standards in support of quality care as needed. They will also ensure collaboration with patients, caregivers, medical directors and members of the IDT.

Requirements

• Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
• Nurse Practitioner's license in the state of operations
• 1+ years experience working within Family Practice, General Practice, Geriatric care or related field
• A valid driver's license and a reliable source of transportation
• Willingness to travel
• Commitment to ongoing professional development and clinical excellence.

Nice To Haves

• Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.

Responsibilities
