As an Advanced Practice Nurse at Health First Loveland, you'll have the opportunity to make a meaningful impact on the health and well-being of patients across all stages of life while practicing in a collaborative, patient-centered primary care environment. Working alongside experienced physicians, nurses, and support staff, you will provide comprehensive care, build lasting relationships with patients and families, and help drive positive health outcomes within the community. This role offers the autonomy to practice at the top of your license, the support of an established healthcare system, and the satisfaction of delivering high-quality, compassionate care in a growing and welcoming community. This position operates under appropriate supervision on all issues relating to patient care and medical practice as required by the Ohio Board of Nursing. Provides the highest quality direct patient care, medical direction and coordination of professional medical care within the practice in an efficient, cost effective, and accountable manner. May manage patients by providing primary health care and case management, focusing on disease prevention, wellness, and management of minor, chronic, and acute illness. Provides holistic care, taking into account the needs and strengths of the whole person. Collaborates with clients and other interdisciplinary health care members to establish realistic goals and activities to guide both the client and the provider in implementing care. This position is knowledgeable of the departments Quality Plan and population management program and actively supports the plan and program through daily activities including but not limited to education to other clinical support staff as needed.
