About The Position

Great new Career Opportunity in Palliative Care! Now Hiring - Advanced Practice Nurse - Bridge Care- (Bergen County Area, NJ). The Advanced Practice Nurse will be responsible for conducting visits to patients on palliative care in accordance with local, state and federal regulations. They will communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care. This role also involves facilitating access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment), consulting with other members of the interdisciplinary team to contribute to the patient's plan of care, and preparing and maintaining accurate patient records, charts and documents. Additionally, the role may assist in the development of clinical practice guidelines/standards in support of quality palliative care as needed and ensure collaboration with patients, caregivers, medical directors and members of the Interdisciplinary Team (IDT).

Requirements

  • Master's Degree in Nursing
  • Nurse Practitioner's license in the state of operations
  • 2-4 years experience working within hospice or related field
  • Must have a Master's Degree
  • Must have a valid driver's license and a reliable source of transportation
  • Must be willing to travel
