About The Position

The Advanced Practice Provider provides in-home longitudinal primary care for the adult population. An APP home visit affords high risk patients access to proactive, comprehensive, compassionate, and collaborative care to improve their health and well-being, while keeping them home. The Advanced Practice Nurse will function within the scope of practice as defined by the state nurses practice act, delineation of privileges set forth by the practice setting and the American Nurses’ Association Code of Ethics. The advanced practice nurse utilizes the scientific process and national standards of care as a framework for managing patient care. As licensed independent providers, APPs practice autonomously and/or in consultation with a collaborating/supervising physician and other health care professionals to assess, diagnose, treat and manage patients’ health problems/needs.

Requirements

  • Minimum of 3 years of experience working with the adult and geriatric population as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant).
  • Current knowledge and understanding of Primary Care, Chronic Care Management, and Best Practices.
  • Master's Degree - Nurse Practitioner or Physician Assistant is required.
  • American Heart Association BLS certification is required.
  • NJ-APN & NJ-RN or NJ PA License is required.
  • Valid State Driver’s License with a clean DMV record.
  • Copy of recent Vehicle Insurance required.

Nice To Haves

  • Home based Primary Care experience preferred.

Responsibilities

  • Perform a patient history and physical examination to determine a working differential diagnosis and develop an individualized plan of care.
  • Ordering, performing, supervising, and interpreting diagnostic and laboratory tests.
  • Prescription of pharmacologic agents and non-pharmacologic therapies.
  • Provide therapeutic measures as appropriate.
  • Provide teaching, education and counseling to individuals, families and groups.
  • Formulate diagnoses in consultation with his/her collaborating/supervising physician for uncommon or unstable conditions and either refer to a specialist or participate in the dual management and treatment with his/her collaborating/supervising physician.
  • Emphasize health promotion and disease prevention.
  • Serve as health care researcher, professional educator, interdisciplinary consultant and patient advocate.
  • Prescribe medications where clinically indicated, in accordance with current APP prescribing guidelines and in accordance with state and federal law.
  • Proper documentation of medications prescribed, and refills authorized, will be reflected in the patient’s chart.
  • Stay current with medical trends and provide evidence-based practice medicine by completing the required continuing education as mandated by certifying organizations.
