About The Position

Doctors at Home is seeking Full and Part-time Advanced Practice Nurse Practitioners to join our growing Primary Care House Calls practice in Arlington/Grand Prairie/Mansfield, TX. Applicants must live in Arlington and be able to commute to the patients' homes. This role involves performing physical exams and patient observations for Geriatric patients in various home settings, recording medical histories, creating care plans, ordering and analyzing diagnostic tests, diagnosing and treating complex health issues, managing medications, and consulting with healthcare professionals and families. Training patients and their families on illness and injury prevention is also a key responsibility. The position offers no on-call duties, no weekends, and no major holidays.

Requirements

1 year experience as a NP caring for Geriatric patients, preferably in the home environment is required.
Must be able to speak, read, write and comprehend English proficiently.
Applicants must live in Arlington and able to commute to the patients' homes.

Responsibilities

Performing physical exams and patient observations for Geriatric patients in their home, ILF, ALF, Memory Care, Group Homes and Senior Communities.
Recording patients' medical histories and symptoms in EMR system.
Creating patient care plans and contributing to existing ones.
Ordering, administering, and analyzing diagnostic tests.
Identifying the need for and ordering medical equipment.
Diagnosing and treating complex health issues.
Managing, ordering, assessing effectiveness of medications and other treatments.
Detecting changes in patients' health and modifying treatment plans, as needed.
Consulting with healthcare professionals and families.
Training patients and their families to manage and prevent illness and injury.
