Annual compensation between $135,000-$150,000, PLUS an annual performance-based, discretionary incentive. Compensation is dependent on non-discriminatory factors including but not limited to an applicant's skills, education/degrees, certifications, prior experience, market data, and other relevant factors.

Requirements

• Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
• Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
• Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements.
• Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.
• MVR with reliable transportation and clean driving record.
• DEA license and BLS certification
• High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
• Must be a self-motivated professional who enjoys working independently and making a meaningful impact on member health.
• Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
• Ability to document assessment findings accurately and thoroughly
• Maintain patient, team member and employer confidentiality while remaining in compliance with all HIPAA regulations.
• Demonstrated clinical skills necessary to perform in-home assessments

Nice To Haves

• Preferred two years of experience in a primary care or longitudinal care setting.

Responsibilities

• Deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care.
• Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews.
• Manage chronic conditions and develop individualized treatment plans.
• Maintain accurate EMR documentation and comply with healthcare regulations.
• Educate patients on disease management and healthy living.
• Collaborate with interdisciplinary teams to ensure coordinated care.
