About The Position

Jean Tweed Centre is a leading, community-based organization that provides a safe and supportive environment for women with substance use, mental health, and gambling concerns. Recognizing the diversity of women’s experience in today’s society, our programs seek to address a range of issues in a woman’s life that may contribute to substance use and gambling concerns. Understanding each woman’s experience within the broader physical, emotional, social, cultural and gender framework, we provide a client-centered approach to health. This program will improve Jean Tweed’s integrated treatment program bridging women’s transition from intensive treatment to community life. The Aftercare Counsellor will offer evidence-informed aftercare groups (e.g., SMART Recovery, DBT, MBRP) and provide follow-up support for up to six months post-discharge through calls and check-ins to reduce isolation, connect clients to community resources, and track outcomes.

Requirements

  • Master’s degree in Social Work, Psychology, Counselling, or related field.
  • Registration in good standing with relevant regulatory body (e.g., Ontario College of Social Workers and Social Service Workers, College of Registered Psychotherapists of Ontario).
  • 3 years of experience in the field of substance use, mental health and trauma-informed practice.
  • Experience working with women who have been involved in the shelter system, criminal justice system, or had difficulty accessing appropriate health services.
  • Group facilitation and prgram development skills
  • Ability to work collaboratively within multidisciplinary teams
  • Strong computer skills including working knowledge of Microsoft Office
  • Knowledge of community resources and supports in the Toronto area.
  • Ability to prepare and present information in both formal and informal settings.
  • First Aid/CPR certification.

Responsibilities

  • Provide evidence-informed aftercare groups for clients completing an intensive program
  • Launch weekly aftercare groups for clients completing an intensive program
  • Provide follow-up support for up to six months post-discharge through calls and check-ins to monitor recovery progress, identify potentials relapse risks and reduce isolation for clients
  • Provide referrals for supporting care and follow-up with institutions to promote seamless integrated care
  • Identify and make accessible a variety of supports including, housing, financial aid, family support, crisis intervention/prevention/support, advocacy for women in need
  • Identify and build linkages with agencies, other supporting organizations and social service systems in the area to promote consistent and effective intervention for clients.
  • Maintain documentation of relevant client interactions and interventions, and contacts with collateral supports.
  • Monitor the progress of clients, prepare reports and letters as necessary.
  • Develop and maintain attendance tracking and evaluation tools
  • Collaborate with internal teams and external service providers to coordinate care and referrals.
  • Participate in case conferences, supervision, and professional development opportunities as agreed to with Supervisor.
  • Engage in ongoing staff development and personal learning as agreed to in performance reviews.
  • Collaborate with supervisor to discuss job performance and establish professional goals.
  • Conduct job responsibilities with a commitment to harm reduction, trauma- informed, women-centred and anti-oppressive practices.
  • Abide by the Occupational Health and Safety Act, and work in a manner that is safe, reporting incidents immediately to direct supervisor.
  • Adhere to the policies and procedures of The Jean Tweed Centre.
  • Perform other duties as assigned in order to meet the overall goals and objectives of The Jean Tweed Centre.
