About The Position

knownwell is a weight-inclusive healthcare company dedicated to transforming obesity care. We offer a comprehensive range of services including weight management, primary care, nutrition counseling, and health coaching, delivered through a hybrid model of in-clinic and virtual support. With significant funding and a mission to expand access to evidence-based obesity care nationwide, we are rapidly growing. As an Advanced Practice Clinician (APC) at knownwell, you will collaborate with a multidisciplinary team of physicians, APCs, registered dietitians, registered nurses, and registered pharmacists. Your role will focus on providing comprehensive medical care, with a specialization in the diagnosis, treatment, and prevention of obesity and related conditions within a primary care setting. This position offers a unique opportunity to significantly impact patients' lives by improving their overall health.

Requirements

Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA).
Unrestricted DEA licensure and prescribe authority (per state practice regulation).
At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred.
If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification.
Active, unrestricted license to practice in California, or willingness to obtain.
Willingness to obtain additional licensures if needed based upon business needs.
Excellent communication and interpersonal skills.
Dedicated to staying current with the latest developments in primary care and obesity medicine.

Responsibilities

Practice exceptional medicine within knownwell
Sees adults for primary care, often with a focus on metabolic health, or adults for metabolic health only.
Share a panel with MD/DO + other APCs.
Embodies the knownwell mission and values of patient inclusivity in how you deliver care.
Coordinates with the broader care team to ensure the patient has the support they need.
Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.
Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.
See adolescents ages 12+ for metabolic health and primary care.
Ensure panel is open to any individual, regardless of their current health status and comorbidities.
Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey.
Provide an environment in which patients feel comfortable and free from judgment.
Offer exceptional encouragement as patients reach their goals.
Coordinate across the collaborative care team
Escalate any concerns to your dedicated site leadership team.
Provide clinical guidance to medical assistants and nursing teams as assigned.
Stay current with licensure and appropriate re-certifications.
