About The Position

knownwell is a weight-inclusive healthcare company dedicated to changing how obesity care is delivered. We offer a comprehensive care model combining in-clinic and virtual support for weight management, primary care, nutrition counseling, and health coaching. With significant funding and a mission to expand access to evidence-based obesity care nationwide, we are growing rapidly. As an Advanced Practice Clinician (APC) at knownwell, you will collaborate with a multidisciplinary team including physicians, registered dietitians, nurses, and pharmacists. Your role will focus on providing comprehensive medical care, with a specialization in the diagnosis, treatment, and prevention of obesity and related conditions within a primary care setting. This position offers a unique opportunity to significantly improve patient health outcomes.

Requirements

  • Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA).
  • Unrestricted DEA licensure and prescribe authority (per state practice regulation).
  • At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred.
  • If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification.
  • Active, unrestricted license to practice in Illinois, or willingness to obtain.
  • Willingness to obtain additional licensures if needed based upon business needs.
  • Excellent communication and interpersonal skills.
  • Dedicated to staying current with the latest developments in primary care and obesity medicine.
  • Belief in inclusive, compassionate care.
  • Belief in shared decision making with a patient. Honor the goals a patient has for their health, and partner with them to help them achieve those goals.
  • High emotional intelligence and ability to make a patient feel at ease – without judgment and without shame.
  • Willingness and excitement to try new technologies that are both patient and provider oriented.
  • Excited to work with a collaborative team of physicians, APC’s, registered dietitians, medical assistants, health coaches, and other staff.
  • Authorized to work in the United States (visa sponsorship not currently available).

Responsibilities

  • Practice exceptional medicine within knownwell.
  • See adults for primary care, often with a focus on metabolic health, or adults for metabolic health only. Share a panel with MD/DO + other APCs.
  • Embody the knownwell mission and values of patient inclusivity in how you deliver care.
  • Coordinate with the broader care team to ensure the patient has the support they need.
  • Document as necessary to ensure accurate billing, including review of remote patient monitoring data.
  • Communicate with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.
  • See adults ages 18+ for primary care and metabolic health.
  • See pediatric patients ages 12+ for metabolic health only, if appropriately licensed and experienced.
  • Ensure panel is open to any individual, regardless of their current health status and comorbidities.
  • Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey. Provide an environment in which patients feel comfortable and free from judgment.
  • Offer exceptional encouragement as patients reach their goals.
  • Coordinate across the collaborative care team.
  • Escalate any concerns to your dedicated site leadership team.
  • Provide clinical guidance to medical assistants and nursing teams as assigned.
  • Stay current with licensure and appropriate re-certifications.
