About The Position

Patients with advanced heart failure need a physician who can change the trajectory of disease, not just manage the next admission. In this role you will lead evaluation, therapy selection, and longitudinal care for people facing mechanical support, transplant, or complex medical management. Your judgment will determine who is listed, who is bridged, and who recovers enough to stay off both. You will practice where cardiology, surgery, and critical care meet, and where outcomes are measured in survival, graft function, and days lived outside the hospital.

Requirements

  • MD or DO, ABIM Cardiovascular Disease certification, and board certification or eligibility in Advanced Heart Failure and Transplant Cardiology.
  • Completion of an ACGME-accredited advanced heart failure and transplant fellowship, with hands-on MCS and post-transplant management.
  • Unrestricted state medical license (or eligibility) and ability to obtain hospital privileges and DEA registration.
  • Demonstrated participation in transplant selection committees and comfort with UNOS listing criteria and regulatory documentation.
  • Evidence of safe independent call coverage for cardiogenic shock, device alarms, and early post-operative transplant care.

Responsibilities

  • Evaluate and risk-stratify referred patients with Stage D heart failure and produce a clear treatment plan—medical optimization, durable MCS, or transplant—within program timelines.
  • Lead inpatient advanced-HF and post-transplant care so that length of stay, readmission, and early graft complications meet or beat program benchmarks.
  • Select and manage temporary and durable mechanical support in partnership with cardiac surgery, including candidacy, complications, and weaning or destination decisions.
  • Build and maintain a transplant waitlist that reflects accurate urgency, complete workups, and documented multidisciplinary consensus.
  • Direct outpatient follow-up for MCS and transplant recipients, adjusting immunosuppression and HF therapy to protect graft function and reduce rejection and infection.
  • Contribute to quality review, UNOS/regulatory readiness, and protocol updates that measurably improve listing-to-transplant efficiency and one-year survival.
  • Teach fellows and advanced-practice clinicians so the service can cover call and clinic without gaps in decision quality.
