About The Position

The Advanced Community Care Clinician is a clinical role responsible for delivering medical care in the communities where Cityblock members live in homes, shelters, offices, public places or in the street. The ACCC/AC3 is primarily responsible for facilitating virtual provider visits with Cityblock members and following medical orders, performing clinical skills and tasks within their scope, training, and policy, which may include IV insertion, IV/IM/SQ medication administration, 12 EKG, Phlebotomy, etc. (Dependent upon local legal regulations). The Advanced Community Care Clinician is pivotal in connecting members to Cityblock services including medical providers and access to all Cityblock services. Requirements

  • Current EMT Paramedic certification required
  • 4+ Years of work experience

Responsibilities

  • Facilitating virtual provider visits with Cityblock members
  • Following medical orders
  • Performing clinical skills and tasks within their scope, training, and policy
  • Connecting members to Cityblock services including medical providers and access to all Cityblock services
  • Conducting Emergency at home visits for acute illness facilitated provider visits
  • Conducting Facilitated Visits for longitudinal care team facilitated provider visits
  • Conducting Outreach visits to find hard to reach members
  • Conducting Specimen Collection visits to obtain tissue samples for the lab
  • Conducting MIC Only Followup visits with a specific set of visit goals
  • Reviewing equipment, communications, and scheduled visits at the beginning of each shift
