About The Position

The Physician House Calls (PHC) Physician Assistant / Advanced Practice Provider (APP; Nurse Practitioner (NP) or Physician Assistant (PA)) provides comprehensive primary medical care for the Physician House Calls (PHC) program to home bound older adults in their own residence under the guidance of the Vice President of Medical Services and Chief Medical Officer in collaboration with other members of the medical and interdisciplinary team. This position requires a high degree of independence, personal responsibility, and collaboration with other members of the interdisciplinary team, patients, their caregivers/family, and other service providers. This program aims to bring person-centered care to frail older adults in their place of residence and minimize the risk of requiring emergency services and hospitalization.

Requirements

  • Nurse Practitioner (Master or Doctorate degree or equivalent) in Geriatrics, Family or Adult primary care.
  • Physician Assistant (Master degree or equivalent) with relevant experience and current, unrestricted license to practice in New York State.
  • Current, unrestricted New York State Driver License, automobile and automobile insurance required.
  • Current and appropriate required certification.

Nice To Haves

  • Minimum of two years of experience as an APP, ideally with a background in geriatrics, primary care, palliative care, and/or long-term care.
  • Demonstrated ability to function well in a team as well as independently.
  • Previous telehealth experience.
  • Use of electronic medical records.
  • Comfort with mobile technology.

Responsibilities

  • Delivering excellent direct clinical care including routine, urgent, and follow-up visits to patients in their place of residence and via telehealth.
  • Engaging in interdisciplinary care planning and collaboration with the PHC RN Program Coordinator, other members of the interdisciplinary care team, and other service providers.
  • Evaluating laboratory, diagnostic results, and consultative reports to inform patient care assessments.
  • Providing education, support, and counseling to patients and their caregivers/families.
  • Documenting care delivery in accordance with professional standards and ensuring accurate billing for services.
  • Completing documentation, billing, and orders in a timely fashion.
  • Record time for chronic care management (CCM) in a timely fashion.
  • Equitably participating in on-call service for evening, weekend, and holiday by phone.
  • Ensuring high-quality medical care in collaboration with the healthcare team.
  • Contributing to customer service initiatives and satisfaction measures.
  • Adhering to health, medical and quality improvement policies in line with organizational and New York State Department of Health regulations.
  • Engaging in medical research, scholarly activities, or special projects as directed.

Benefits

  • $115,000-$122,000 plus potential for quarterly productivity incentive bonuses
