About The Position

The Orlando Health Arnold Palmer Pediatric Bone Marrow Transplant & Cellular Therapy Program provides highly specialized care for children from newborn through early adulthood requiring cellular therapy for malignant and non-malignant conditions, including pediatric cancers, blood disorders, and immunodeficiencies. Our multidisciplinary team delivers comprehensive inpatient and outpatient care across the BMT continuum. We are seeking a dedicated and experienced Advanced Practice Provider (APP) to join our growing team. Position Summary: The Advanced Practice Provider (APP), in collaboration with attending physicians and the BMT Program Director, provides comprehensive care to pediatric bone marrow transplant patients. Responsibilities include history taking, physical assessment, review of diagnostic studies, development of individualized treatment plans, and management of acute and chronic conditions related to BMT care. Clinical responsibilities span both inpatient and outpatient environments, with active involvement in patient education, care coordination, and program development. The APP will also participate in procedures including lumbar punctures, bone marrow aspirates and biopsies, and aid with bone marrow harvests, as well as the acute care management of inpatient coverage

Requirements

  • Graduate of an accredited physician assistant program or has obtained an MSN or APRN degree from an accredited nursing school.
  • Acute Care Nurse Practitioner certification or Primary Care Pediatric Nurse required
  • Current Florida licensure as APRN or PA-C.
  • Board certification via PNCB or NCCPA.
  • Maintain active BLS, PALS.

Nice To Haves

  • Chemotherapy and Biotherapy certification preferred.
  • Preferred minimum 3 years of experience as an RN or APP in Pediatric Hematology-Oncology, Bone Marrow Transplant, Neuro-Oncology, Solid Tumor, Liquid Tumor, Leukemia and Lymphoma, General Oncology, Bleeding Disorders.

Responsibilities

  • Outpatient Pediatric BMT Clinic
  • Provide comprehensive evaluation and management of pediatric patients in collaboration with the Pediatric BMT physician. This includes new patient assessments, hospital and post procedure follow-ups, and symptom-based consultations. Be available for ‑same day and ‑next day appointments to address acute patient concerns or complications. Perform administrative responsibilities such as documentation, scheduling, and coordination of clinic operations, while triaging phone calls and facilitating‑ communication with community physicians and specialists. Support satellite clinics and specialty clinics as needed. Educate and counsel patients and families on diagnoses, treatment options, pre‑ and post‑ admission/procedural care, and preventive strategies, while actively engaging in multidisciplinary care coordination. Contribute to ongoing program development, service quality, and care process improvements.
  • Inpatient Pediatric BMT Service
  • Participate in daily rounds with the BMT medical team and provide comprehensive inpatient and outpatient evaluation and management of BMT patients across both the BMT unit and the 4thfloor Hematology/Oncology inpatient unit. Conduct new patient consults, manage admissions, and perform essential inpatient clinical duties. Manage acute complications related to BMT treatment, including transplant related toxicities and other emergent conditions. Develop, update, and implement individualized patient care plans. Oversee inpatient orders and outpatient prescriptions, including chemotherapy, immunosuppressants, and infusion therapies. Coordinate home care needs, discharge planning, and patient and family education. Perform procedures such as lumbar punctures, bone marrow aspirations and biopsies, and assist with bone marrow harvests. Communicate effectively with consulting services, multidisciplinary teams, and allied health professionals. Provide cross coverage with Hematology/Oncology APPs as needed to ensure service continuity and high-quality patient care.

  • Demonstrate a strong understanding of the indications for autologous and allogeneic hematopoietic progenitor cell (HPC) transplantation, along with the processes involved in donor and recipient selection, evaluation, and informed consent. Responsibilities also include familiarity with preparative regimens, cellular therapy product administration, and the use of adjunctive therapies such as growth factors.
  • Management of Transplant-Related Complications
  • Manages transplant related complications, including those associated with cellular therapy product administration, neutropenic fever, thrombocytopenia with bleeding, and hemorrhagic cystitis. Addresses considerations for ABO incompatible HPC products and associated transfusion requirements such as CMV negative and irradiated blood products. Evaluates and manages pulmonary complications, fungal infections, sinusoidal obstruction syndrome, dermatologic toxicities, hepatic dysfunction, acute and chronic graft versus host disease (GvHD), and neurologic, cardiac, renal, and allergic or anaphylactic complications. Identifies and manages cytokine release syndrome, tumor lysis syndrome (TLS), and macrophage activation syndrome/hemophagocytic lymphohistiocytosis (MAS/HLH).
  • Monitoring & Supportive Care
  • Provides ongoing monitoring and supportive care for complications such as mucositis, gastrointestinal disturbances, and pain, while also addressing infectious complications associated with immunodeficiency and opportunistic pathogens.
  • Evaluation Responsibilities
  • Assess the late effects of cellular therapy and conducting posttransplant follow-up to monitor long-term survivorship outcomes.
  • Documentation & Reporting
  • Ensures accurate documentation for investigational protocols and reports for all adverse events according to institutional and regulatory requirements.
  • Additional competencies
  • Knowledge of palliative and end of life care principles, age specific donor and recipient considerations, and the methodology and clinical implications of HLA typing and chimerism assays.

  • Call Requirements
  • Overnight call coverage is currently provided by the Hematology/Oncology/BMT attending physician and the pediatric resident. The APP is responsible for daytime weekend shift rotations and participates in rotating holiday coverage in collaboration with inpatient Hematology/Oncology APPs. Call expectations may evolve over time in response to program growth and patient care needs.
  • Quality Improvement, Research & Education
  • Attend and contribute to educational conferences, unit-based quality initiatives and continuing education activities to improve patient care and promote evidence-based practice. Support the education of students, residents, advanced practice provider fellows, and new team members when applicable.
  • Operations
  • Attend and participate in HemOnc/BMT Section meetings as well as any assigned BMT departmental or program meetings. The APP is also encouraged to attend the APH Pediatrics Department Meeting, medical PIPS review, and Pediatric Grand Rounds.
  • Paid Time Off (PTO)
  • Requests for PTO must be made at least 30 days in advance according to OH policy. However, operational needs may require additional notice. PTO requests are granted on a first come first serve basis.
  • Holiday Coverage
  • Holidays are rotated evenly amongst BMT/HemOnc APPs. Holidays are defined as follows by OH policy: 3 minors include Memorial Day, July 4th, and Labor Day. Majors include Thanksgiving Day, Christmas Day, and New Year’s Day.
  • Work Hours
  • Outpatient clinic hours are generally Monday through Friday. When rotating to the inpatient service, work hours and days will vary based on scheduled weekend coverage. Weekend coverage will be incorporated into the APP’s regular weekly schedule, with flexed time off provided during the week when possible. In situations where schedule flexing is not feasible, additional compensation may be provided for extra shifts
