About The Position

Value-based APP role with $7,500 sign on package. This position involves working Monday - Friday, from 8 AM to 4:30 PM, with no weekends or evenings. The role focuses on managing chronic conditions with a patient load of 6-8 patients per day. An Athena AI Scribe is available to assist with charting within the workday. The patient panel geography will cover Round Rock, Georgetown, Belton, Jarod, and extend up to Temple. Requirements

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record

Nice To Haves

• Geriatric training/experience preferred
• Experience in home health, geriatrics, hospice and primary care

Responsibilities

• Provide primary care to high acuity patients in their home Monday - Friday
• Manage chronic conditions
• Practice medicine with a focus on patient care, not volume
• Take the time needed to truly address the patient’s needs
