About The Position

Provides assessment and case management services to elderly and disabled adults who are at risk of abuse, neglect, or exploitation and Provides support to the Adult Protective Services case managers and coverage of the Adult Protection Services intake line.

Requirements

  • A bachelor’s degree from an accredited institution with a major in a human behavior science field, OR a degree with 30 semester hours, or 45 quarter hours, of qualified work in development in human behavioral sciences or health care related courses, such as, social work, sociology, psychology, psychiatry, gerontology, nursing special education, family intervention techniques, diagnostic measures, therapeutic techniques, guidance and counseling, criminal justice, or other human behavioral sciences, or a medical field relevant to the APS program and/or at-risk adults.
  • One year of professional casework in a public or private social services agency completed after the degree is obtained.
  • Any combination of education, training and experience which provides the knowledge, skills and abilities required for the job.

Responsibilities

  • Provides one-on-one assessments of at-risk adults who are unable to protect their own interests and makes an evaluation based on physical appearance and hygiene; medical, nutritional, and environmental needs; competency and ability to understand self-determination and the result of the situation that is placing him/her at risk; activities of daily living; support systems; ability of the client to manage financial affairs.
  • Seeks court action to obtain protective services in emergency situations.
  • Performs case planning for individuals who require ongoing protective services which may include referral for services to reduce harm and risk and the use of intervention tools with the client and/or client support system.
  • Evaluates services delivery system.
  • Provides coordination of assessments and/or ongoing service delivery with appropriate collateral agencies.
  • Performs monthly contact visits for ongoing investigation, assessment, and implementation of services.
  • May be asked to cover cases when the assigned case worker is unavailable.
  • Documents assessments and actions taken through all phases of the case.
  • Educates the community and organizations in recognition and prevention of adult abuse and neglect.
  • Communicates effectively with multiple teams within DHS, community agencies, clients, and families.
  • Takes calls regarding mistreatment of at-risk adults and enters the reports into the appropriate database.
  • Searches various databases to gather additional client information to enhance the screening process.
  • Coordinator for special projects as needed.
  • Oral and written communication must be clear and effective.
  • Performs other duties as assigned.
  • Provides administrative support for the Social Case Workers in Adult Protection Services.
  • May offer and refer services for elderly and disabled adults at least 18 years of age who are at risk from current or possible abuse, sexual abuse, exploitation, and/or neglect by self or others.
  • Assists Social Caseworkers by requesting documents from law enforcement, doctor’s offices, nursing home, and service providers.
