About The Position

Responsible for efficiently and accurately coordinating the care between patients and other service providers within Crawford County Mental Health Center (CCMHC), as well as in the community to assist in improving the functioning and quality of life which are due to social determinants of health, physical health condition and mental illness.

Requirements

  • Graduation from high school or equivalency is required.
  • A valid driver’s license is required.
  • Pass pre-employment background checks.
  • Obtain Certification for Management of Aggressive Behavior (MOAB) training.
  • MOAB training requires controlled movements, body positioning, and the ability to perform physical holds and restraint techniques.
  • First Aid CPR Training.

Nice To Haves

  • A bachelor's degree from an accredited university in the various behavioral health fields such as clinical psychology, social work, counseling, nursing, rehabilitation or closely related degree, is preferred.
  • Experience in helping professionals and occupations is desired.

Responsibilities

  • Follow dress code policy of any alternative work locations, including but not limited to schools, jails, courts, detention centers, other community partners, etc.
  • Accurately obtain and record patient National Outcome Measures (NOMs) information in the EHR
  • Assess clients’ needs by identifying demographics, historical information, assessments including BMI, Columbia, and SDOH.
  • Comply with all clinical documentation standards relevant to care provisions in timelines required by policy.
  • Provide outreach via telephone and travel within the community, to provide assistance to clients and their families regarding access to health, elimination barriers, and improving social determinants of health.
  • Arrange medical, social, educational, and other services with providers as required by treatment plans, and as assigned, including care coordination functions.
  • Maintain a client caseload up to 450 clients at a frequency determined by a level of care system.
  • Knowledge of local resources and qualifications to make referrals and follow up that the appointment was kept 80% of the time
  • Complete hospital admission and discharges outreach within 24 hrs. of learning of the event.
  • Track and refer patients for basic health prevention including a primary care provider, labs, and recommended screenings based on development and age, yearly.
  • Provide vitals and documentation in the EHR a minimum of yearly.
  • Assist clients to apply for patient financial assistance such as Beacon (Carelon), Medicaid, Patient Assistance, or Social Security
  • Follow up with patients who missed or cancelled an appointment to help with rescheduling, including problem-solving barriers to treatment.
  • Meet department productivity and documentation standards.
