About The Position

Ascentria's mental health case management services provides support to individuals living in the community. Case Managers assist adults with mental health challenges to live independently. The work is done out in the local community and case managers can substantially impact the lives of the individuals they work with. The Adult Mental Health Case Manager plays a crucial role in identifying, assessing, and evaluating services for individuals, ensuring their health and safety to promote independent living. Their focus is on recovery strategies, community support, and advocating for client rights while encouraging self-advocacy.

Requirements

  • A High School Diploma or GED is mandatory.
  • A minimum of 6 months of relevant experience is desired.
  • A valid state-issued driver's license and proof of current insurance are required.
  • Must possess an MHRT-C or MHRT-C Provisional certification and maintain it with 18 approved CEUs every two years.
  • Must successfully complete all required background checks before starting their employment.

Nice To Haves

  • A related associate's degree is preferred.
