About The Position

Center for Life Management (CLM) provides comprehensive outpatient mental health services for adults and elders, focusing on individuals with severe and persistent mental illness, co-occurring developmental disabilities, and substance use disorders. The Adult Community Support Counselor offers outpatient, community-based support to help clients improve symptoms and behaviors that hinder daily functioning and personal wellness goals. This is a full-time position requiring 35 hours per week.

Requirements

  • Bachelor's degree preferred, or an Associate's degree with a minimum of two years of direct service in a related field.
  • Ability to exercise sound clinical judgment in routine, urgent, and emergent situations.
  • Proficient computer skills, including Microsoft Office applications.
  • Ability to document effectively and efficiently.
  • Ability to navigate an electronic health record.
  • Ability and desire to work as part of a clinical team, as well as be self-directed with minimal supervision.

Nice To Haves

  • Must be a regular, full-time CLM employee in good standing for 9 months or more to be eligible for the Master's degree program.
  • Must hold a Bachelor's Degree to be eligible for the Master's degree program.
  • Must agree to a five (5) year commitment of full-time employment with CLM, starting from the program participation date, to be eligible for the Master's degree program.
  • Participants in the Master's degree program must maintain a B average.
  • Employee is responsible for the applicable taxable portion of tuition remission for the Master's degree program.

Responsibilities

  • Provide counseling and Functional Support Services (FSS) in home and community settings, aligned with client treatment plan objectives.
  • Deliver Individual Therapeutic Behavioral Services (TBSI) to build and reinforce coping strategies for symptom and behavior management.
  • Offer Family Training and Counseling (FTC) to educate natural supports on promoting client resiliency and recovery.
  • Provide Medication Training and Support (MTS) to educate, problem-solve, and develop strategies for medication adherence.
  • Perform Crisis Intervention (CI) during acute symptom exacerbations to ensure client safety.
  • Collaborate with treatment team members, natural supports, and external providers for a wrap-around approach to client care.
  • Provide a set number of direct service hours weekly to meet the 60% productivity KPI, with potential for incentive pay.
  • Serve on relevant committees as assigned by supervisor.
  • Maintain professional conduct, protecting patient integrity, confidentiality, and rights.
  • Execute all required services and documentation according to agency policies and standards.
  • Maintain professional memberships, licenses, accreditation, and certifications.
  • Represent CLM professionally in all settings.
  • Perform other duties as assigned by supervisor.
