About The Position

Center for Life Management (CLM) provides comprehensive outpatient mental health services for adults and elders, focusing on individuals with severe and persistent mental illness, co-occurring developmental disabilities, and substance use disorders. The Adult Community Support Counselor offers outpatient, community-based support to help clients improve symptoms and behaviors that hinder daily functioning and personal wellness goals. This is a full-time position requiring 35 hours per week.

Requirements

Bachelor's degree preferred, or an Associate's degree with a minimum of two years of direct service in a related field.
Ability to exercise sound clinical judgment in routine, urgent, and emergent situations.
Proficient computer skills, including Microsoft Office applications.
Ability to document effectively and efficiently.
Ability to navigate an electronic health record.
Ability and desire to work as part of a clinical team, as well as be self-directed with minimal supervision.
Ability to acquire and practice new clinical skills as needed.
Effective communication skills.
Organizational skills.
Ability to multi-task.
Reliable transportation.
Clean driving record.
Proof of auto insurance coverage.

Nice To Haves

Must be a regular, full-time CLM employee in good standing for 9 months or more to be eligible for the Master's degree program.
Must hold a Bachelor's Degree to be eligible for the Master's degree program.
Must agree to a five (5) year commitment of full-time employment with CLM, starting from the program participation date, to be eligible for the Master's degree program.
Participants in the Master's degree program must maintain a B average.
Employee is responsible for the applicable taxable portion of tuition remission for the Master's degree program.

Responsibilities

Provide counseling and Functional Support Services (FSS) in home and community settings, aligned with client treatment plan objectives.
Deliver Individual Therapeutic Behavioral Services (TBSI) to build and reinforce coping strategies for symptom and behavior management.
Offer Family Training and Counseling (FTC) to educate natural supports on promoting client resiliency and recovery.
Provide Medication Training and Support (MTS) to educate, problem-solve, and develop strategies for medication adherence.
Perform Crisis Intervention (CI) during acute symptom exacerbations to ensure client safety.
Collaborate with treatment team members, natural supports, and external providers for a wrap-around approach to client care.
Provide a set number of direct service hours weekly to meet the 60% productivity KPI, with potential for incentive pay.
Serve on relevant committees as assigned by supervisor.
Maintain professional conduct, protecting patient integrity, confidentiality, and rights.
Execute all required services and documentation according to agency policies and standards.
Maintain professional memberships, licenses, accreditation, and certifications.
Represent CLM professionally in all settings.
Perform other duties as assigned by supervisor.
