About The Position

Morrison Center is seeking a dedicated and compassionate Adult Case Manager for their Oakland, Maine location. This role focuses on empowering adults with intellectual and developmental disabilities through comprehensive, individualized support services. Morrison Center is a private, non-profit organization committed to making a difference in the lives of individuals with developmental disabilities.

Requirements

Bachelor’s degree in any field.
A minimum of 1 year of paid experience working with individuals with intellectual and/or developmental disabilities (e.g., DSP, BHP).
Strong organizational, writing, and time management skills.
Ability to thrive in a fast-paced environment with multitasking and problem-solving responsibilities.
Proficiency in Microsoft Office Suite.

Nice To Haves

Previous experience in adult case management is preferred but not required.
Some transportation is required.

Responsibilities

Complete home visits and program visits.
Manage a caseload of 22-25 clients.
Provide one-on-one support to adults in a community setting.
Facilitate team meetings.
Complete person-centered assessments, comprehensive assessments, and 90-day reviews.
Develop and implement individualized support plans using a person-centered approach.
Analyze services and support strategies to ensure they meet client needs and goals.
Guide and advocate for clients navigating community resources, supports, and opportunities.
Maintain accurate and timely documentation in compliance with DHHS regulations.
Serve as a liaison between clients, families, and community partners.
Represent Morrison Center with professionalism and compassion.
