About The Position

Under the direct supervision of the Health and Human Services (HHS) Director, the Case Manager is responsible for responding to the targeted community needs and support of the mission, philosophy and goals of Ak-Chin Health and Human Services. This position is responsible for ensuring the integration of best practices in social work. This position also requires the flexibility to work beyond normal work hours including nights, weekends, and holidays.

Requirements

Bachelor's Degree from an accredited college or university in Social Work, Public Health, Psychology, Education, or a closely related field and three (3) years of professional level work experience in social work: OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
Previous experience coordinating the work of others or leading a team.
Must possess a valid Arizona driver's license and be able to meet the Community's insurance carrier requirements.
Must successfully complete an extensive fingerprint/background check.
Must be alcohol and drug free.
Knowledge of history, culture, laws, rules, customs and traditions of the Ak-Chin Indian Community.
Knowledge of Social Work principals and concepts.
Knowledge of behavioral health work principles and concepts.
Knowledge of Bureau of Indian Affairs Welfare Assistance Grant Programs.
Knowledge of the State of Arizona Division of Developmental Disabilities (DDD) system, Arizona Health Care Cost Containment System (AHCCCS), Arizona Long Term Care System (ALTCS), benefits such as Medicare and Medicaid, and Social Security Administration (SSA).
Knowledge of the economic, educational, and social service needs and problems in the Community.
Knowledge of crisis intervention methods.
Skill in training subordinate staff.
Skill in case management including planning, scheduling, and time management.
Skill establishing and maintaining effective working relationships with other social work professionals, community officials and clients.
Skill in evaluating customer needs and utilizing Ak-Chin Indian Community resources or other appropriate referrals to meet client needs.
Skill dealing with crisis situations.
Skill dealing with sensitive and confidential personal issues and maintaining appropriate professional, confidential relationships.
Ability to communicate effectively verbally and in writing.
Ability to maintain accurate, up to date files, records and reports.
Ability to work under pressure with multiple task deadlines.
Ability to maintain effective working relationships with other employees, Community officials and the general public.
Working knowledge of HIPPA requirements and release of information request.
Ability to use personal computers, related software and other standard office equipment such as facsimile, photocopier and calculator.
Ability to drive Ak-Chin Indian Community vehicles.

Responsibilities

Provides intake services for referrals for General Assistance, Burial Assistance, Homemaker Services, Emergency Assistance, Adult Protective Services and Adult Institutional Care.
Provides follow up and support services to individuals and their families.
Refers client to other resource agencies and programs such as, but not limited to, Behavioral Health and Health Services.
Coordinates with other agencies to provide emergency assistance and resources involving food, clothing, crisis intervention, and/or medical services.
Coordinates, assigns and schedules work of the Homemaker staff.
Trains new and current homemaker staff.
Provides feedback to the HHS Director on Homemaker work performance and conduct, and assists Director in addressing performance or conduct issues.
Reviews Homemaker timesheets for accuracy and submits for processing.
Conducts home visits or visits to other venues to provide direct casework and case management services to individuals and families.
Coordinates long-term care services including home and Community based services for elderly and disabled.
Establishes and maintains a network of elderly and disabled resources for referral purposes, both on and off the Ak-Chin Indian Community.
Responsible for planning and scheduling activities.
Intervenes in crisis emergencies, including "On Call" crisis intervention assistance.
Maintains regular office hours in order to be available to clients and other Community members.
Participates in training opportunities that will enhance professional knowledge of Tribal case management, adult abuse, neglect and proper service methodologies for the older adult and disabled population.
Participates in staff meetings and discusses issues or cases that require additional input and planning.
Provides alternative solutions to issues to ensure greatest benefit for the client in accomplishing HHS department objectives.
Develops Individual Self-Sufficiency Plans for clients.
Responds to referrals for adults in need of care and conducts inquiry to determine referral validity.
Maintains current and comprehensive client case files.
Meets with other tribal, state, and federal programs that provide services to clients.
Works with local medical programs to coordinate client services.
Performs other work-related duties as assigned by the HHS Director.
