We are launching an innovative pilot program aimed at managing post-hospital discharge patients through a hybrid model that integrates in-person consultations, virtual visits, and telehealth services. The client is in search of a Physician Assistant (PA) who is enthusiastic about contributing to a dynamic program with significant long-term prospects. Key highlights of this opportunity include: A newly established program designed for expansion and development. Outpatient care model with no on-call responsibilities. A balanced approach to patient care, encompassing both clinic-based and virtual environments. Initial assignment of 6 months, with potential for extension based on program needs and performance.
Requirements
- Associate's degree in Physician Assistant studies
- Current certification as a Physician Assistant (PA-C)
- Strong clinical assessment and diagnostic skills
- Excellent interpersonal and communication abilities
- Ability to thrive in a fast-paced, collaborative environment
- Familiarity with electronic health record (EHR) systems
- Commitment to patient-centered care
- Strong problem-solving skills and adaptability
Responsibilities
- Conduct comprehensive health assessments for adult patients
- Develop and implement personalized treatment plans
- Collaborate with physicians and other medical staff to optimize patient care
- Supervise and support nursing staff in daily operations
- Provide education and counseling for patients and families
- Document patient interactions accurately and timely in medical records
- Participate in continuous education and training programs
- Assist in managing chronic diseases and acute conditions
