Adult Care Coordinator/Case Manager (MHRT/C) - $1,500 Sign-On Bonus
About The Position
The Adult Care Coordinator (ACC) provides services focused on the specific needs of adults with behavioral health diagnoses who have a decreased ability to function independently at home and in the community. The ACC develops client-centered individual service plans and ensures services are approved through the Administrative Service Organization (ASO) Managed Care System. The role also maintains current benefit and financial information for clients with cognitive disabilities to help ensure their benefits remain applicable. The position follows a regular office schedule five days per week, Monday through Friday, between 6 a.m. and 5 p.m., and requires travel within a designated area. The position is based in Whitefield, Maine, and is full-time, with an hourly salary range of $22.00 to $25.00.
Requirements
- Be at least 18 years of age.
- High school diploma or equivalent.
- MHRT/C or MHRT/CP credential.
- Intermediate-level computer experience.
- Valid Maine driver's license.
- Proof of vehicle insurance at $50,000/$100,000.
