About The Position

Open Roads, a department of A Safe Haven, is a leading provider of comprehensive behavioral health services. We are seeking a compassionate and dedicated Licensed Clinical Social Worker/Licensed Clinical Professional Counselor to join our team. Open Roads serves children, adolescents, young adults, and their families through Intensive Outpatient Programs (IOP) and traditional outpatient therapy. We also partner with a public charter school to provide weekly individual and/or group therapy on-site with high school students. This role will provide therapy services in three high schools located on the South Side of Chicago. Our mission is to provide quality, evidence-based treatment to historically disenfranchised communities and clients. In this role, you will have the opportunity to provide support and therapy services to youth and their families struggling with behavioral health and substance use challenges. Requirements

  • Be available to be on-site at identified schools Monday-Friday during school hours.
  • Master's degree in psychology, social work, counseling, or a related field from an accredited program.
  • Current licensure as a Licensed Professional Counselor (LCPC) or Licensed Clinical Social Worker (LCSW) in the state of Illinois.
  • A minimum of 3 years of clinical experience providing therapy and counseling services to individuals with behavioral health disorders, preferably in a community mental health, outpatient settings, or schools.
  • Experience in working with adolescents and/or family clients.
  • Strong clinical assessment and diagnostic skills, with the ability to formulate accurate clinical impressions and treatment plans based on client needs.
  • Proficiency in delivering evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), Acceptance & Commitment Therapy (ACT), motivational interviewing (MI), and trauma-informed care.
  • Education, training, and experience with one or more of the following: mood disorders, anxiety, trauma, substance use, eating disorders, and/or externalizing behaviors.
  • Excellent communication and interpersonal skills, with the ability to establish rapport with clients, build therapeutic alliances, and collaborate effectively with colleagues and stakeholders.
  • Commitment to cultural competence, diversity, equity, and inclusion, with the ability to work effectively with clients from diverse backgrounds and identities.
  • Ability to work independently and as part of a team in a fast-paced, dynamic environment, demonstrating flexibility, adaptability, and resilience.
  • Knowledge of ethical and legal standards governing the practice of social work and counseling, including confidentiality, informed consent, and mandated reporting requirements.

Responsibilities

  • Conduct comprehensive psychosocial assessments to evaluate clients' needs, strengths, and goals.
  • Develop individualized treatment plans in collaboration with clients, utilizing evidence-based therapeutic modalities and interventions to address behavioral health issues.
  • Conduct individual and/or group therapy sessions with youth to address mental health issues, such as mood disorders, anxiety, and substance abuse.
  • Engage with families in consultations or therapy sessions to address family dynamics, communication, and other issues that may be impacting the adolescent’s mental health.
  • Coordinate care for adolescents, including developing treatment plans, monitoring progress, and adjusting plans as necessary.
  • Work with families, schools, and other involved parties to ensure comprehensive care.
  • Work alongside other mental health professionals, including psychologists, case managers, and counselors to provide coordinated care.
  • Participate in regular team meetings to discuss cases and treatment strategies.
  • Maintain accurate and timely records of all therapy sessions, assessments, treatment plans, and communications with other professionals.
  • Ensure that all documentation complies with state and federal regulations as well as organizational policies.
  • Assist with the development of a discharge plan when adolescents are ready to transition out of treatment.
  • Ensure continuity of care, make referrals to appropriate services, and prepare the youth and their family for the transition.
  • Stay abreast of current research, best practices, and trends in the field of behavioral health and social work.
  • Engage in ongoing professional development activities, including training, supervision, and peer consultation, to enhance clinical skills and knowledge.
  • Provide therapy services in three high schools on the South Side of Chicago.
  • Transition school clients to virtual or on-site appointments at the clinic during summer months.
  • Manage a caseload of 24-32 individual, family, and/or group appointments.
