About The Position

The Clinical Nurse provides direct, comprehensive nursing care to patients across the continuum of care in a high-acuity, fast-paced environment. The role requires rapid clinical assessment, sound judgment, and adherence to professional nursing standards. The nurse plans, implements, and evaluates patient-centered care, safeguards patient rights, and collaborates with interdisciplinary teams to achieve optimal clinical outcomes. Practice is aligned with organizational values, quality improvement principles, and regulatory requirements. The nurse identifies patients appropriate for discharge, reviews physician orders, and initiates admission and discharge processes. The role ensures prescriptions, follow-up appointments, and discharge paperwork are completed; coordinates with case management, pharmacy, and other departments to facilitate timely discharges; and supports patient throughput and discharge goals. The nurse provides individualized education about diagnoses, medications, treatment plans, side effects, symptom management, and post-discharge care, and assesses patient understanding using teach-back methods. The position requires accurate, timely documentation using electronic medical record systems and adherence to documentation standards that support continuity of care, quality monitoring, and accreditation reporting. The nurse communicates and escalates barriers to admission, discharge, or patient progression; promotes patient safety and reduces avoidable delays in care transitions; and maintains compliance with TGH policies, regulatory requirements, and evidence-based practices. Ongoing professional development and maintenance of specialty knowledge are expected.
