St. Francis Hospice (SFH) is growing, and we need an Admissions Registered Nurse to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.
Requirements
- Associate Degree in Nursing (ADN) required.
- Current and active Florida Registered Nurse (RN) License required.
Nice To Haves
- Bachelor of Science in Nursing (BSN) preferred.
- Hospice, home health, palliative care, or related experience preferred.
Responsibilities
- Determining the eligibility and preparation of patient/family need and the initial written plan of care.
- Working closely with the Intake Coordinator, Clinical Service Department, and the Chief Operating Officer to ensure timely processing of all referrals.
- Serving as a resource for questions/issues from the health care community and SFR staff.
