Admissions RN : FCS Hospice - Hoag at Home
About The Position
The Admissions RN organizes and directs hospice care/home health and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. This role involves reviewing hospice/home health qualification criteria, providing an overview of services to patients and families, completing and maintaining accurate clinical notes, recording pain/symptom management changes, and communicating with physicians regarding patient needs and changes in condition. The schedule for this role is Thursday - Monday from 11:00 AM - 7:30 PM.

Responsibilities
- Organizes and directs hospice care/home health.
- Builds from community resources to plan and direct services to meet the needs of individuals and families in their homes and communities.
- Reviews hospice/home health qualification criteria and provides an overview of hospice/home health services to the patient and their family.
- Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given.
- Records pain/symptom management changes/outcomes as appropriate.
- Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
- Performs other duties as assigned.
