This position coordinates the intake of referrals, obtains patient information and physician orders, performs admissions of appropriate patients, establishes initial plans of care, assists with assignments of patients to field nurses for ongoing case management, assists with team conferences, ensures that patient care records are accurate and complete, shares clinical expertise with nursing staff, coordinates services with the social worker, and contributes to the performance improvement program of Hospice of Lubbock. The role requires minimal supervision. As a Charge/Lead, this position assists in the coordination of clinical services for Hospice of Lubbock, ensuring coordination with referrals, obtaining patient information and physician orders, performing admissions of appropriate patients, establishing initial plans of care, assisting with assignments of patients to field nurses for ongoing case management, assisting with team conferences, ensuring that patient care records are accurate and complete, ensuring coding of patient diagnoses, sharing clinical expertise with nursing staff and other team disciplines, and contributing to the performance improvement program of Hospice of Lubbock. This role also serves as the Alternate Nursing Supervisor when the Clinical Manager is unavailable and requires minimal supervision.
