About The Position

Emmanuel Hospice is a mission-driven, non-profit hospice organization dedicated to providing holistic, compassionate care to individuals with life-limiting illnesses. We are currently seeking a caring and experienced Admissions Registered Nurse (RN) to join our team and serve as the welcoming presence for patients and families beginning their hospice journey. Your work sets the tone for the entire hospice experience, helping patients and families take the first step on their hospice journey with dignity and support.

Requirements

Current RN license in the state of Michigan
Minimum of 2 years clinical experience, preferably in hospice, palliative care, or acute care
Exceptional interpersonal and communication skills
Ability to explain medical and hospice-related information with compassion and clarity
Strong organizational and documentation skills
Valid driver’s license, reliable transportation, and willingness to travel locally

Responsibilities

First clinical contact for patients and families entering their hospice journey while providing emotional support and reassurance
Conducting initial hospice information sessions and admissions in patients’ homes, hospitals, or facilities
Performing education, clinical assessments and collaborating with physicians on hospice eligibility
Clearly explaining hospice services, goals, and benefits to patients and their families
Completing all required documentation and consent forms in a timely manner
Coordinating with the interdisciplinary team to ensure a smooth transition into hospice services
