About The Position

Trinity Hospice Dallas is seeking a PRN Admissions RN to join their team. This role is ideal for an experienced RN with a background in hospice care. The position offers a stipend plus pay per visit and requires availability on Saturdays and Sundays, with some weekday work also expected. Admissions are completed before 7 PM. Requirements

Current Registered Nurse license in the state of Texas.
At least two (2) years of experience as a RN Case Manager in the hospice setting.
Current Texas Driver's license.
Dependable transportation.
Experience as a RN for a hospice agency or program.

Responsibilities

Administer skilled nursing for all ages in their place of residence.
Conduct patient admissions.
Complete assessments to determine patient eligibility for hospice services.
Co-market with Business Development Executives/Marketers.
Assist with deaths.
Assist with patient visits as needed.
Complete periodic inservices.
Coordinate care within a hospice interdisciplinary team to meet patient/family needs.
Communicate with attending physicians, hospice physicians, and other involved physicians.
Demonstrate accountability, flexibility, respect, and integrity.
Comply with applicable legal requirements, company standards, policies, and procedures.
Timely submit all required paperwork.
