About The Position

BAYADA Home Health Care is seeking compassionate and experienced Per Diem Weekend Admissions Registered Nurses (RNs) to join our Essex County team. In this specialized role, you'll complete OASIS Start of Care (SOC) visits for adult and geriatric patients, helping them begin their home health journey with exceptional clinical care and personalized support. Requirements

  • Current New Jersey Registered Nurse (RN) license in good standing.
  • Minimum of one year of nursing experience, preferably in home health.
  • Comfortable using electronic medical records; HCHB experience is a plus.
  • Strong assessment, communication, organizational, and time management skills.
  • Ability to work independently while providing exceptional patient care.

Nice To Haves

  • OASIS experience and Start of Care (SOC) experience preferred.

Responsibilities

  • Complete OASIS Start of Care (SOC) admissions for adult and geriatric home health patients.
  • Perform comprehensive nursing assessments and develop individualized plans of care.
  • Deliver skilled nursing interventions, including medication management, wound care, treatments, and patient education.
  • Monitor patient conditions and communicate changes with physicians and interdisciplinary team members.
  • Educate patients and caregivers to promote safety, independence, and successful management of health conditions at home.
  • Utilize critical thinking skills to coordinate care and identify patient needs.
  • Accurately document assessments and services using electronic medical records.
