About The Position
Join AmeriCare Plus In-Home Personal Care Agency as an experienced Registered Nurse (RN) to lead client admissions and assessments. This role involves conducting in-home assessments, developing personalized care plans, coordinating admission paperwork, ensuring compliance, and communicating with families, caregivers, and physicians. The goal is to support clients with compassion and clinical expertise during their transition to in-home care, enabling them to live safely and comfortably at home.
Requirements
- Active RN license in Virginia
- Minimum 1 year of nursing experience
- Strong assessment and communication skills
- Reliable transportation and valid driver's license
Nice To Haves
- Home health or admissions experience preferred
