About The Position

St. Joseph Hospice is hiring a Hospice Admissions Registered Nurse in Mobile, Alabama. The position is scheduled from 11 a.m. to 7 p.m. and requires patient visits at patients’ residences or facilities, as well as participation in the agency’s on-call schedule, including call once a week as needed. The RN assesses patients, contributes to hospice care planning and education, coordinates with patients, families, physicians, and the hospice team, and maintains accurate medical records. Requirements

Current Registered Nurse (RN) license in the State of Alabama.
Current CPR certification.
Previous hospice experience.
Knowledge of electronic medical records (EMR).
Compassion and strong time-management skills.

Responsibilities

Perform initial and ongoing patient assessments.
Assist in developing and implementing the Hospice Plan of Care.
Educate patients and families about the Plan of Care, including medication administration, dressing changes, oxygen administration, hospice philosophy, and care of terminally ill patients.
Serve as the primary liaison between the physician, patient and family, and hospice team.
Maintain accurate and updated medical records.
Make routine supervisory visits.
Participate in the agency on-call schedule; call is required once a week as needed.
Follow company policies and procedures, compliance guidelines, applicable laws, and regulations, and promptly report suspected compliance violations.
Make patient visits at patients’ residences or facilities.
